Measurement of drugs and their metabolites in biological fluids is the foundation of both therapeutic drug monitoring (TDM) and toxicology. Each discipline depends on accurate identification and quantification if the measurements are to be useful. The LC-MS/MS is pervasive in the large laboratories and increasingly so in the mid-tier toxicology and clinical settings because of its robustness, versatility and economics. Now, forward-looking laboratories are adopting the LDTD-MS/MS configuration because LDTD further increases throughput, as well as drives down costs sample-to-sample basis.

Given the limitations of immunoassays, most labs use a two-step process, starting with a qualitative screen, and follow with a confirmation to identify the individual drugs or compounds.
